Duration: February, 2020 – April 2022 Location: 10 selected districts in Punjab Funded by: World Diabetes Foundation Activities: To strengthen 84 rural health centers (in 10 districts) for diagnosis, treatment and reporting of type-2 diabetes care, as per program protocols To link 84 rural health centers with respective district-headquarter hospital (for specialist care) and community-based vaccination activity for healthy life style promotion To design and conduct method-mix evaluation of offering integrated diabetes care at rural health centers, including linking with hospital and community-based care activity Expected Results: Train more than 350 doctors and 250 allied staff for an integrated diabetes care in ten selected districts Link 84 rural health centers with forty specialist units (i.e. medicine, surgery, obstetric and eye units) in ten district hospitals through IT bases Linkage Enable vaccinators associated with 84 rural health centers for community-based healthy life style promotion sessions 9,000 diabetes cases (at least half are women) get diagnosed, registered and treated during the project life At least 1,800 specialist consultations arranged at respective district headquarter hospitals 4,500 community-based healthy life style promotion sessions conducted
